Photo/Video Release Form – I grant to Right to Life of Northeast Indiana the right to take photographs or videos of me in connection with the above-identified organization. I authorize Right to Life of Northeast Indiana, its assignees and transferees, to copyright, use, and publish the same in print and/or electronically.

I agree that Right to Life of Northeast Indiana may use such photographs or videos of me with or without my name and for any lawful purpose, including, for example, such purposes as marketing, publicity, illustration, advertising, and Web content.

Code of Conduct (please read thoroughly, as this will not be gone over during orientation)
1. Chaperones – Chaperones are here to help you. If you need ANYTHING, don’t hesitate to go to one of them or to Phoebe, the event coordinator. If it’s the middle of the night and you need something, feel free to reach out to your head chaperone, whose number is on your name tag.
2. Schedule – Please stick to the schedule you’re given at check-in to help with safety and chaperone coverage. It's posted in the dorms and also outside of each of the classrooms. Please be on time to each activity. If you get lost or something happens, Phoebe’s phone number is on the back of your lanyard, so you can call or text her.
3. Dorms and Lights Out – Inside the dorms, girls may not go into the guys’ dorm area, and guys may not go into the girls’ dorm area. The lounge and lobby are open for you to hang out. Lights out is at 11:00pm, with activities ending at 10:30pm. Students are expected to stay in their rooms after lights out, barring emergencies and needing to use the bathroom. Head chaperones will be notified if students are outside their rooms unnecessarily or going into others’ rooms after lights out.
4. Keys – If you lose your dorm key and swipe key card, it will cost you $25 (this is the amount USF will charge us for any missing keys).
5. Property Boundaries – You can hang out outside, but you must stay within the vicinity of Bonzel Dorms and on the front side of the building, unless you are at one of the scheduled activities in a different location. Generally, if you’re near water or a road, turn around; you’ve gone too far. If you need to leave campus for some reason during camp, you will need to get approval from the camp director (Phoebe).
6. Food – Meals will be at the cafeteria. If you have a food allergy and are having trouble finding foods that work for you, please let a chaperone know so they can get you what you need. You can’t learn on an empty stomach! We also have snacks provided throughout the next three days. Please do your part to keep snack areas clean!
7. Respect – We have top-of-the-line instructors for you this weekend. Treat them and your classmates with courtesy. This camp is for you to learn, so we encourage you to ask questions and interact with each other and the instructors, but you must do so kindly and respectfully. This courtesy also extends to the chaperones who are assisting with camp. Also, you all know how to use your phones appropriately during class. If a chaperone sees inappropriate behavior, you may be asked to leave the classroom.
8. PDA – There will be no public displays of affection. You’re here to learn, as is everyone else, so that needs to be your focus.
9. Video Games – We do not allow video game consoles on campus during camp due to issues with them in the past. If you are found with one, it will be confiscated for the remainder of the camp and returned upon your departure.
10. Code of Conduct Repercussions – We take our code of conduct seriously. If you break the code of conduct and it is deemed minor misconduct, you’ll be given a warning. However, if the misconduct is repeated, or if it is deemed too extreme to receive only a warning, parents will be called and asked to come pick up students, regardless of time of day.

By signing, I acknowledge that I am the parent or legal guardian of the above student and that I give him/her permission to attend the Life Defenders Training on the campus of the University of St. Francis. I have read and understand the description of this event, and I grant permission for emergency medical treatment on the recommendation of two medical doctors. I release Right to Life of Northeast Indiana & Three Rivers Educational Trust Fund and all other sponsors and staff from any liability resulting from illness or injury.
